From Bob Landrys Little Black Book of Tags. Notated in "number system" format, with X=Do and 2, 3, 4, etc denoting position in the major scale. No sharps or flats, an underline pushes the note up a 1/2. No sight-reading necessary. Over the years, I think people have added the lead swipe down with the bari. I really like that version, but this is the one that was in the book! (And how often does the baritone get to gliss down an entire octave?) IMHO, one of the best bari tags ever.
